Summary:
Power up or wake up revs fans to high speed. Two consecutive restarts put them back to normal.
Despite weeks of searching the web for solutions, I have yet to come across a fan speed problem quite like this, and none of the suggested solutions for fan problems have worked.

While asleep, nothing unusual happens, but upon wake up (or start up, if I shut it down) the fans immediately rev to high speed (within a second or so).
A restart from the login panel will usually (not always) bring the system to an intermediate state, where the fans aren't racing, but are still faster than normal with speeds varying a lot.
A second restart usually (not always) brings everything back to normal (good fan speeds and temperatures), and it will run this way indefinitely until another sleep or shutdown cycle.
Except for the fan speed problem, there are no other indicators of problems.
Things I have tried include:
Resetting the PRAM
Resetting the SMU by unplugging for several minutes Pressing the SMU reset button Running reset-nvram, set-defaults, & reset-all from Open Firmware Disconnecting all external devices except keyboard & mouse Blowing out dust (it was already pretty clean) Trying various processor performance settings Disabling the power button sleep option Deleting the PowerManagemant.plist file Running the Hardware Test CD (all tests pass) Running the TechTool Pro (three versions - all tests pass) Cleaned caches with Onyx Replaced the battery
Any more ideas? I'm going to try reinstalling Mac OS next, but have little hope for that. I've also read that reinstalling firmware might help, but can't get the installer to do it (says it's already up to date).

My system is a Dual 2.5 GHz Power Mac G5 (June 2004) with 1.5 GB DDR SDRAM, analog speakers, iSight, and AlchemyTV card. Power on self-tests pass. All other hardware works normally. There have been no hardware configuration changes to the system in years.

I'm using Mac OS 10.4.11 with all the latest updates. Firmware is version 5.1.8f7. Hard crashes have been extremely rare -- none since several weeks prior to this strange behavior.
